How do I join BGSU-FA?

To join the BGSU-FA, you must complete a BGSU-FA Dues Authorization Form (aka “Membership Form”). Choose ONE of the following methods for accessing and completing the Membership Form:

• Complete and submit a Membership Form online by clicking HERE

  • You will be asked to submit your name and BGSU email to access the DocuSign system.

  • After submitting this information, you will receive a signing validation code via email.

  • Copy and paste your signing validation code as prompted; you will be able to complete the Membership Form online via DocuSign.

• Download a copy of the Membership Form by clicking HERE

  • Check the boxes where indicated.

  • Provide your contact information and signature

  • Scan the completed form and email it to lauren.maziarz@bgsu-fa.org.

  • You should retain retain a copy of your completed form for your records, and return the original via regular mail to the following address:

Your national AAUP dues are included in the dues you pay to the BGSU-FA.
